To request a copy of your Provisional BHP, SBBHP, or BHP certificate, please complete this electronic Certificate Request Form.

Before submitting a request, we encourage you to contact your former employer or the Certified BHP Instructor who taught your class, and search your own records to locate your certificate.

The following information is required to locate archived certificates. You must complete all of the required fields and as many of the additional fields as possible.

Due to the large number of requests, please allow 7 to 10 business days for processing.
